On November 18, 2021, the National Medical Commission (NMC) published a milestone gazette notification titled Foreign Medical Graduate Licentiate (FMGL) Regulations, 2021. This document fundamentally changed the rules for Indian students pursuing medical education abroad.
To this day, hundreds of students enrol in non-compliant foreign courses every year because unscrupulous local agents fail to explain these rules. Here is a clear, legal breakdown of what FMGL 2021 mandates.
1. Rule 1: 54-Month Minimum Academic Course Duration
The NMC mandates that the medical course duration must be a minimum of 54 months (4.5 years). Courses that run for 3.5 or 4 years are legally INVALID for medical registration in India.
2. Rule 2: 12-Month Compulsory Clinical Internship at the SAME Institution
Students must complete a 12-month continuous clinical internship in the hospital attached to the foreign medical university where they studied. You CANNOT complete your foreign internship in India or at an unapproved third-party clinic.
3. Rule 3: 100% English Medium of Instruction
The entire course of education, clinical training, and examinations must be delivered in English. Bilingual courses (e.g. 3 years English + 3 years Russian/Kazakh) do NOT fulfill NMC guidelines.
4. Rule 4: Subject Curriculum Alignment
The foreign medical university curriculum must include mandatory clinical subjects: Anatomy, Physiology, Biochemistry, Pathology, Microbiology, Pharmacology, Forensic Medicine, Community Medicine, General Medicine, Pediatrics, Psychiatry, General Surgery, Orthopedics, ENT, Ophthalmology, OBG, Anesthesia, Radiology, and Dermatology.
5. Rule 5: Legal Medical Registration in Host Country
The foreign medical degree must grant the graduate official legal registration as a medical practitioner in that country, equivalent to domestic graduates.
6. Rule 6: CRMI Internship in India Post-FMGE
After returning to India and qualifying in the FMGE/NExT exam, foreign graduates must complete a 1-year Compulsory Rotating Medical Internship (CRMI) in an Indian hospital to receive full permanent registration.
